About the Role

The Palliative Care Nurse is responsible for promoting the services and expertise of hospice and palliative care to the community, patients, and family members. This experienced registered professional nurse initiates and coordinates the palliative care plan, provides skilled palliative care, and coordinates services for patients. The nurse works cooperatively with the attending physician, Palliative Medical Director, and other members of the interdisciplinary Palliative Care team, participating in patient care conferences.

The Palliative Care Nurse provides care to patients diagnosed with a life-limiting illness, regularly reviewing and updating the plan of care. Responsibilities include performing prescribed medical treatments, pain management, symptom control, conducting admissions, assessments, and evaluations, and providing education and supportive care to patients and caregivers. The nurse acts as Case Manager for Palliative Care patients and supervises the Palliative Care LVN.

Responsibilities

  • Initiate and coordinate the palliative care plan for patients.
  • Provide skilled palliative care and coordinate services for patients.
  • Collaborate with attending physicians, Palliative Medical Director, and interdisciplinary team members.
  • Participate in patient care conferences.
  • Perform prescribed medical treatments, including pain management and symptom control.
  • Conduct admissions, assessments, and evaluations of patients.
  • Provide education and supportive care to patients and caregivers.
  • Act as Case Manager for Palliative Care patients.
  • Supervise and oversee the Palliative Care LVN.
  • Promote hospice and palliative care services to the community.

Requirements

  • Current Professional Nursing License in California.
  • At least one or more years of hospice experience.
  • Knowledge of hospice and palliative care requirements, including Medicare, Medi-Cal, and private insurance guidelines.
  • Strong communication skills, both oral and written.
  • Good marketing skills.
  • Ability to respond to the needs of physicians, discharge planners, patients, and family members.
  • Valid driver’s license, reliable transportation, and auto insurance.

Qualifications

  • Bachelor’s degree in nursing preferred.
  • Certified Hospice and Palliative Nurse (CHPN) preferred.
