Hospice RN - Full Time

About the role

Evaluates patients, develops and implements individualized care plans, and collaborates with an interprofessional team to provide compassionate hospice care. When serving as case manager, coordinates all aspects of patient care.

Responsibilities

• Assess patient preferences and promote cultural sensitivity
• Manage changes in patient conditions
• Ensure evidence-based practices for quality outcomes
• Strong communication and critical thinking
• Adhere to safety standards

Requirements

• Registered Nurse (RN) license and a valid driver’s license
• Minimum of 2 years nursing experience; preferred 2-5 years in home or hospice care
• Nursing diploma or associate's degree required; bachelor's degree preferred

Qualifications

• Experience in supporting diverse and vulnerable populations
• Focus on pain and symptom management, counseling, and bereavement services is preferred
