Hospice RN - Full Time

About the role

We are seeking a Registered Nurse (RN) Case Manager to provide in-home hospice care, assessing and managing patients' physical, psychosocial, emotional, and spiritual needs.

Responsibilities

• Conducting initial evaluations within 48 hours of referral
• Developing and implementing personalized Plans of Care
• Coordinating with patients, families, and healthcare professionals
• Documenting patient progress
• Reassessing patients at least every 15 days
• Providing education on self-care
• Ensuring quality care delivery in diverse home environments

Qualifications

• Current RN license in the state
• Valid driver's license
• Reliable transportation
• Preferably, one year of clinical experience
• CPR certification or ability to obtain it within 90 days (preferred)
