Hospice RN - Full time

About the Role

Provides specialized nursing care to hospice patients and supports their families, assessing patient needs, developing and implementing individualized care plans, and collaborating with an interprofessional team. When acting as case manager, coordinates all aspects of patient care, ensuring effective communication and adherence to evidence-based practices. Promotes respect, empathy, and cultural sensitivity in interactions with diverse populations.

Responsibilities

• Assess patient preferences, barriers, and holistic needs
• Develop, document, and communicate personalized care plans
• Implement and evaluate nursing interventions, adjusting plans as needed
• Collaborate with team members, patients, and families for optimal care
• Ensure safety and quality standards are met in all care activities

Requirements

• RN license with a valid driver’s license
• Associate's degree in nursing (Bachelor's preferred)
• Less than 2 years of experience; 2-5 years preferred for advanced roles
• Strong communication, cultural competence, and critical thinking skills
• Ability to work safely and maintain alertness in a safety-sensitive environment
