Pediatric Palliative Care RN
About the role
Under the direction of the Palliative Care team, the Palliative Care RN is responsible for individual patient/family assessment, facilitating patient care, and performing patient and family education in conjunction with Palliative Care medical providers. Assessment includes complex medical, emotional, social, spiritual, and financial needs, and assisting families and the healthcare team to create a comprehensive care plan based on family goals. The role regularly communicates with Social Services to help facilitate access to community-based and Providence resources, serves as the hub of multiple lines of communication between the patient, referring physician, palliative care physician, and inpatient/outpatient support services providers, and assesses and facilitates appropriate treatment per orders for patients at end of life on “Comfort Care.” The position encompasses nursing assessment and liaison abilities.
