Pediatric Medically Complex Care Nurse Navigator

Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.

Responsibilities

• Provide quality education, follow-up, and emotional support to patients and their families.
• Case manage the population throughout the inpatient and outpatient settings and measure outcomes to maximize continuity of care between the settings.
• Manage projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes.

Requirements

• Bachelor's Degree in Nursing (Required)
• Master's Degree in Nursing (Preferred)
• 5 years of previous nursing or CDE clinical experience
• Prior teaching experience with patients of all ages

Qualifications

• LIC-Registered Nurse (RN) - State of Michigan (Required upon hire)
• CRT-Basic Life Support (BLS) - AHA or ARC (Required within 90 days)
• CRT-Advanced Cardiovascular Life Support (ACLS) - AHA or CRT-Pediatric Advanced Life Support (PALS) - AHA (Required within 120 days)
