Patient Placement Coordinator RN - Full time Variable
About the role
Patient Placement Center Nurse supports care for Providence St Joseph Health (PSJH) patients by evaluating and coordinating the patient intake process. In this role, you will take and triage calls, facilitate inbound and outbound transfers for PSJH facilities, and act as a liaison between physicians, hospitals, and other care providers who seek to refer, schedule admission, or transfer patients into the hospital. You will ensure timely medical communication for patient referrals and transfers to and from the facility.
You will employ critical thinking to integrate objective data with knowledge gained from preliminary information and ongoing assessment, including patient health care history, for optimal placement. Additionally, you will serve as a clinical resource for the team, providing guidance on clinical concerns, level of care determination, and regulatory considerations. You will also support the onboarding and training of new staff, helping reinforce consistent practices across the teams.
Responsibilities
- Evaluate and coordinate the patient intake process.

Requirements
- Associate's Degree - Graduate from an accredited Associate Degree in Nursing (ADN).
- California Registered Nurse License (upon hire).
Qualifications
- Bachelor’s Degree - Graduate from an accredited School of Nursing (preferred).
- Minimum of 2-5 years of nursing experience.
- 1 year of working knowledge of Epic and Bed management system.
- 1 year of call center experience.
Schedule
Full-time, 10-hour variable shifts.
Pay
Pay range: $57.28 - $88.92 per hour. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
