Patient Placement Coordinator RN - Full Time
Role Overview
This position is a Patient Placement Coordinator RN responsible for evaluating and coordinating patient intake, triaging calls, and facilitating transfers among healthcare facilities. The RN acts as a liaison between physicians, hospitals, and care providers, ensuring timely communication for patient referrals and transfers. He/she employs critical thinking to integrate objective data with patient history for optimal placement. Additionally, the RN provides clinical guidance to the team, supports onboarding and training, and ensures adherence to regulatory standards. The role requires a strong focus on clinical support, communication, and coordination.
Qualifications & Skills
- Associate's Degree in Nursing (ADN) and California RN license required
- Preferred: Bachelor's Degree in Nursing, 2-5 years of nursing experience, knowledge of Epic and Bed management systems, and call center experience
- Strong critical thinking, communication, and clinical guidance skills essential
