RN Patient Placement Coordinator-FT/Variable Hours

About the role

The RN Patient Placement Coordinator ensures continuity of operations by overseeing all patient admissions and transfer activity, monitoring current and prospective capacity, and facilitating patient referrals from outside physicians. This role serves as the central command for patient flow, coordinating throughput from all entry points, and acting as a liaison between clinical departments to secure optimal patient placement while balancing resource constraints and individual needs.

Responsibilities

• Utilizes the nursing process to place patients in the most appropriate bed based on age, spiritual, and cultural needs.
• Serves as the command center for all patient flow within the hospital, including ED, direct admits, same-day surgeries, and transfers.
• Collaborates with the Flow Coordinator/Nursing Supervisor to address flow concerns across the organization.
• Facilitates patient referrals from out-of-area physicians to local providers and arranges transport if needed.
• Acts as a resource for interpreting policies and procedures related to patient placement, including isolation or special care patients.
• Provides bed availability information to Incident Command during disasters.
• Coordinates and communicates system and patient needs with inpatient flow stakeholders.
• Collaborates with healthcare teams to ensure best patient placement.
• Maintains and communicates a global picture of hospital capacity to all departments and physicians.
• Works closely with departments to ensure consistent patient placement processes.
• Communicates frequently with Administrative Supervisors across campuses regarding patient flow and potential issues.
• Notifies the Staffing Office of census fluctuations impacting staffing needs.
• Acts as the central contact for bed management decisions.
• Provides on-call services for PACE and Remote Home Monitoring to support organizational and community services.
• Performs other duties as assigned.

Qualifications

• Associate’s Degree or higher from an accredited School of Nursing.
• Current, active RN license in Virginia or multistate licensure privilege.
• Demonstrated leadership, supervisory, interpersonal, and problem-solving skills.
• Positive attitude and outstanding verbal communication skills.

Preferred Qualifications

• Minimum of two (2) years of clinical nursing experience, with one (1) year in Critical Care.
• Bachelor of Science in Nursing (BSN).

The noise level in the work environment is usually moderate.

Pay

$31.05 - $48.13 per hour.
