Position Highlights
- Position: Patient Navigator
- Location: Chicago, IL
- Schedule: Part time, 20 hours; Monday-Friday, 8am-4:30pm
- Required Travel: No
What You Will Do
- Assist patients with access to social services and community-based resources; document all patient interactions
- Assist patients with scheduling and appropriate referrals; this includes answering telephone calls, returning messages, scheduling appointments, communicating with other hospital departments to manage and obtain referrals
- Maintain inventory of office and medical supplies
- Assist with grant functions as needed; this may include data entry and maintenance, and patient interaction
What You Will Need
- Education: High school diploma or equivalent
- Experience: 1-yr clerical work experience
