PATIENT NAVIGATOR | AMBULATORY CARE SPECIALITY - NEUROLOGY| FULL-TIME DAY SHIFT (25512)

Responsibilities

• Contact patients over the phone to assist in setting up appointments and provide reminder calls prior to every appointment.
• Work with patients, providers, and the BNBMC care team to set goals for patient care.
• Provide navigation for patients by addressing concerns and scheduling complications that could prevent appointment cancellations.
• Address insurance concerns or issues by connecting patients with appropriate resources and team members.
• Provide culturally sensitive services to patients from diverse backgrounds.
• Maintain regular communication with the patient’s providers.
• Refer to internal case management services when other issues are identified.
• Document every intervention into the appropriate medical records.
• Participate in evaluation and research activities for the program as needed.
• Consider social determinants of health in all patient navigation endeavors.
• Perform other duties as assigned to support the overall objectives of the department and organization.

Requirements

• At least 3-5 years of experience working in a community or health setting.
• Proficiency in Spanish preferred.
• Demonstrated commitment to impacting patient care.
• Knowledge of complex patient issues preferred.
• Experience with outreach methods and strategies.
• Bachelor’s degree required.

• Excellent written and communication skills.

• Strong time management, organizational, and planning skills.
• Ability to support, advocate, and coordinate care for clients.

Pay

$42,000 - $60,000 annually.
