Responsibilities
- Contact patients over the phone to assist in setting up appointments and provide reminder calls prior to every appointment.
- Work with patients, providers, and the BNBMC care team to set goals for patient care.
- Provide navigation for patients by addressing concerns and scheduling complications that could prevent appointment cancellations.
- Address insurance concerns or issues by connecting patients with appropriate resources and team members.
- Provide culturally sensitive services to patients from diverse backgrounds.
- Maintain regular communication with the patient’s providers.
- Refer to internal case management services when other issues are identified.
- Document every intervention into the appropriate medical records.
- Participate in evaluation and research activities for the program as needed.
- Consider social determinants of health in all patient navigation endeavors.
- Perform other duties as assigned to support the overall objectives of the department and organization.
Requirements
- At least 3-5 years of experience working in a community or health setting.
- Proficiency in Spanish preferred.
- Demonstrated commitment to impacting patient care.
- Knowledge of complex patient issues preferred.
- Experience with outreach methods and strategies.
- Bachelor’s degree required.
Skills
- Excellent written and communication skills.
- Detail-oriented with the ability to multitask.
- Strong time management, organizational, and planning skills.
- Ability to support, advocate, and coordinate care for clients.
Pay
$42,000 - $60,000 annually.
