Patient Flow Facilitator - Elmwood - Days (6am-6pm) - Full Time

About the role

We operate as a facilitator of centralized psychiatric patient placement and assist with dispatching transportation for patients. This role requires cross-training throughout the department.

Responsibilities

• Responds to system-wide requests for acute behavioral health placement, ensuring all necessary patient referral information is complete, and electronically submits requests for evaluations of patient criteria to determine if admission criteria are met.
• Captures and organizes patient referrals to promote patient safety and meet patient needs within the continuum of care.
• Coordinates transportation for all transport orders by acting as a dispatcher.
• Provides excellent customer service with all customer groups including physicians, nurses, administrators, and colleagues.
• Adapts to change in systems, practices, technology, and variables in practice setting such as patient volume, patient acuity, experience of personnel, utilization of supplies, and resources.
• Rotates between workstations to become competent in each role.
• Communicates pertinent clinical information with referring and admitting providers and administrative staff.

Requirements

• High school diploma or equivalent.
• 1 year of experience in a hospital or clinic business office.

Qualifications

• Proficiency in using computers, software, and web-based applications.
• Effective verbal and written communication skills and ability to present information clearly and professionally.
• Strong interpersonal skills.
• Excellent customer service phone skills and ability to multi-task.
• Working knowledge of medical terminology.
• Ability to be self-directed and motivated and work with little supervision.
• Ability to work extended shifts of 8, 10, or 12 hours per day.
