Patient Care Coordinator for Home Healthcare

About the Role

At Peace Healthcare Agency is looking for an experienced and highly organized Patient Care Coordinator to join our growing home healthcare team. You will play an essential role in making sure our patients receive reliable, compassionate, and well-coordinated care in the comfort of their own homes. You will serve as an important link between patients, families, caregivers, healthcare professionals, and our agency, helping ensure that care needs are addressed and services run smoothly.

Responsibilities

• Coordinate and monitor home healthcare services for assigned patients.
• Communicate regularly with patients, families, caregivers, and healthcare professionals.
• Coordinate caregiver schedules and help ensure appropriate coverage for patient shifts.
• Respond promptly to caregiver call-offs, schedule changes, and urgent staffing needs.
• Follow up with patients and families regarding satisfaction, concerns, and changes in care needs.
• Communicate patient needs and updates to the appropriate clinical or administrative team members.
• Maintain accurate and timely patient and caregiver documentation.
• Track schedules, service hours, care updates, and follow-up requirements.
• Assist with new patient intake and coordination of services when needed.
• Identify and escalate changes in a patient's condition or care needs to the appropriate personnel.
• Help resolve patient, family, and caregiver concerns professionally and promptly.
• Maintain patient confidentiality and comply with HIPAA, agency policies, and applicable regulations.
• Work collaboratively with the intake, staffing, clinical, and administrative teams.

Requirements

We are specifically seeking a candidate with prior experience in home healthcare, home care, patient care coordination, caregiver coordination, scheduling, intake, or a closely related healthcare role. The ideal candidate is:

• Experienced in the home healthcare/home care industry
• Highly organized and detail-oriented
• Comfortable coordinating multiple patients and caregivers
• Excellent at communication and follow-up
• Able to handle last-minute changes and urgent staffing situations
• Compassionate and patient-focused
• Professional when communicating with patients, families, and caregivers
• Able to multitask in a fast-paced environment
• Comfortable using computers, scheduling systems, EMR/EHR, and other office technology
• Knowledgeable about HIPAA and patient confidentiality
• A strong problem solver who can work independently and as part of a team

Qualifications

• Previous home healthcare/home care experience strongly preferred
• Prior experience in patient care coordination, scheduling, intake, staffing, or healthcare administration
• Strong verbal and written communication skills
• Excellent organizational and time-management abilities
• Ability to handle confidential patient information appropriately
• Proficiency with Microsoft Office and healthcare/scheduling software preferred
• High school diploma or equivalent required; healthcare administration or related education is a plus
