Patient Care Coordinator
Become a part of our caring community as a Patient Care Coordinator, working within our local sales team and reporting to a Market Executive of Sales.
About the role
- Coordinate care from Hospitals and Skilled Nursing Facilities (SNFs) in the assigned territory.
- Meet with patients at the bedside to discuss home health services, disciplines, and what to expect at home.
- Support operations teams by ensuring they have the resources, information, and coordination needed for a successful start of care.
- Assist with sales calls as needed to support the Account Executive in addressing client needs and advancing business objectives.
- Travel within the assigned geographical territory is required.
- This role is considered patient-facing and is part of Humana’s Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.
Requirements
- LPN, LVN, or RN license is required.
- Knowledge of governmental regulations, Medicare eligibility requirements, and a comprehensive understanding of potential care plan needs for patients, including coordination of necessary resources.
- Three years of clinical experience preferred.
- Experience with EMR systems such as Watershed and Olio is highly preferred.
Schedule
Scheduled weekly hours: 40 (full time).
Pay
The compensation range below reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The pay range may vary based on geographic location, and individual pay will depend on demonstrated job-related skills, knowledge, experience, education, and certifications.
$59,300 - $80,900 per year
