About the role

Provide compassionate patient care coordination services to ensure seamless transitions between home health services and other healthcare providers.

Responsibilities

  • Coordinate patient care plans with healthcare providers
  • Manage patient referrals and communicate with providers
  • Ensure timely medication management and follow-ups
  • Facilitate communication between patients, families, and healthcare teams

Requirements

  • Bachelor’s degree in Healthcare Administration, Nursing, or related field
  • Minimum 2 years of experience in healthcare coordination
  • Excellent communication and interpersonal skills
  • Proficient in Microsoft Office Suite

Qualifications

  • Current CPR certification
  • Knowledge of home health regulations and standards
