Part Time RN Care Manager

About the Role

This role involves facilitating discharge planning and care coordination from admission to discharge within a collaborative healthcare team, ensuring quality patient care, safety, and efficient throughput.

Responsibilities

• Manage patient transitions, promote seamless care models, and oversee length of stay.
• Intervene with patients with complex or high-risk psychosocial needs as assigned.
• Collaborate with multidisciplinary teams to optimize patient outcomes.

Requirements

• Graduate of an accredited Nursing program with a current state license.
• Bachelor’s Degree of Science in Nursing required.
• 2-3 years of acute care hospital experience preferred; care management or utilization management experience is a plus.
• Certification in BLS (CPR); case management certification preferred.

Skills

• Strong communication, assessment, time management, critical thinking, and advanced computer skills.
• Knowledge of patient-specific care guidelines and ability to interact effectively at all organizational levels.
