RN Care Manager Part Time
About the role
The Progressive Care Unit is seeking a dedicated and experienced Registered Nurse (RN) to join our team. This position involves actively participating in multi-disciplinary rounds, communicating with and educating patients and families, assessing readmitted patients, organizing and facilitating patient and family care conferences, and documenting discharge planning.
Responsibilities
- Participates in multi-disciplinary rounds to review changes in patient status, progression, and level of care, and discharge plans for all assigned patients.
- Educates patients and families regarding emotional, social, and financial impacts of illness and mobilizes family/community resources to meet identified needs.
- Affords patients and families the opportunity to make health care decisions and access needed services.
- Assesses readmitted patients for their perceived reasons for readmission.
- Organizes and facilitates patient and family care conferences with the multidisciplinary team.
- Documents discharge planning evaluation, ongoing assessment, discharge plans, MDRs, barriers to progression of care, avoidable days, and patient and family needs according to standard work.
Requirements
- Associate's degree in Nursing (ADN) or Bachelor's degree in Nursing (BSN).
- Two (2) years of medical/hospital nursing experience.
- Prior experience in Care Management/Utilization Management is preferred.
Qualifications
- Registered Nurse (RN) license.
- Certified Case Manager (CCM) certification is preferred.
- Accredited Case Manager (ACM) certification is preferred.

Schedule
This position is part-time, working 8:00am-4:30pm on Thursdays and Fridays, with Saturdays as assessment-only days. The schedule alternates between Thurs/Fri and Thurs/Fri/Sat.
Pay
The pay range for this position is $33.70 - $62.69 per hour.
