Part-Time RN Care Manager

Responsibilities

• Provide triage, crisis intervention, and skilled nursing care to hospice patients during evenings, nights, weekends, and holidays.
• Coordinate symptom management, pain control, and emergency visits in home and facility settings to ensure patient comfort and dignity.
• Perform phone triage, conduct assessments, implement care plans, and collaborate with the interdisciplinary team.
• Accurately document care activities and offer emotional support and education to patients and families, adhering to hospice philosophy and regulatory standards.
