Part-Time Rehab Nurse (RN)

Role Summary Provide comprehensive nursing care by assessing residents' health, educational, and environmental needs upon admission and throughout their stay. Develop and update individualized care plans in collaboration with an interdisciplinary team, residents, and families. Ensure safe and timely administration of medications, treatments, and interventions, accurately documenting all actions. Utilize critical thinking to monitor residents' conditions, promptly communicating significant changes to physicians using SBARR. Maintain professionalism and composure during crises, addressing issues such as family concerns or staff disputes. Oversee the delivery of care to ensure adherence to care plans, employing QAPI processes for problem resolution. Assist with resident activities of daily living and respond to call lights, supporting residents' well-being. Complete ongoing education to stay current with best nursing practices and organizational policies. Perform other duties as assigned. Qualifications & Skills Registered nurse with an accredited education program and valid license in the state of practiceCPR/AED certification (or willingness to obtain within six months)Critical thinking, strong communication, time management, and organizational skillsPhysical ability to stand, walk, lift, and transfer residents, and operate equipment safelyManual dexterity, visual acuity, and proficiency in documentationPreferred experience in long-term care
