Responsibilities

• Deliver skilled clinical services in diverse home environments
• Document patient visits thoroughly

• Communicate effectively with patients, families, and healthcare team members
• Adapt to varying home settings and serve patients regardless of background

Requirements

• Current, unrestricted LPN license
• Valid driver’s license and reliable transportation

Preferred Qualifications

• CPR certification
• Home care experience
• Strong organizational and communication skills
• Ability to work independently and flexibly
