About the role
In-Home Nurse Practitioner or Physician Assistant responsible for conducting comprehensive in-home health evaluations for health plan members.
Key Responsibilities
- Review medical histories and perform physical exams using digital documentation tools.
- Identify and close care gaps to improve patient outcomes.
- Collaborate with interdisciplinary teams to assess risk factors and support preventive care.
- Educate patients on managing chronic conditions to promote health and wellness.
Qualifications & Skills
- Licensed Nurse Practitioner or Physician Assistant with relevant clinical experience.
- Strong communication and collaboration skills to work effectively with healthcare teams.
- Comfortable using health technology and digital documentation tools.
- Ability to work independently and adapt to flexible schedules.
Key Duties
