Responsibilities

• Conduct comprehensive clinical evaluations for health plan members in their homes.
• Review medical histories, reconcile medications, and identify care gaps using advanced technology.

Qualifications & Skills

• Licensed Nurse Practitioner or Physician Assistant with relevant clinical experience.
