Part Time Home Health Registered Nurse

Key Responsibilities

• Serving as the primary clinical partner, delivering personalized, one-on-one nursing care at patients' homes to promote recovery, safety, and independence.
• Conducting comprehensive in-home assessments to evaluate patients' physical, emotional, and environmental needs, developing patient-centered care plans.
• Performing skilled nursing interventions such as wound care, infusions, medication management, catheter care, and post-operative or chronic disease management.
• Providing education and support to patients and caregivers, empowering them to manage symptoms, medications, and daily care confidently.
• Monitoring patient progress, identifying changes early, and communicating with healthcare teams to adjust care plans and prevent hospitalizations.
• Ensuring care coordination across providers and accurate, timely documentation to support quality and compliance.

Qualifications And Skills

• Active RN license in the practicing state.
• At least one year of clinical nursing experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Valid driver’s license, reliable transportation, and auto insurance.
• CPR certification or ability to obtain it.
