About the role
Provide individualized, in-home nursing care to promote recovery, safety, and independence for patients and families. Conduct comprehensive assessments, deliver skilled interventions such as wound care, infusions, medication management, and post-operative care, and educate caregivers to empower symptom and medication management. Monitor patient progress, communicate with care teams, and coordinate services to ensure high-quality outcomes. Complete accurate documentation to support compliance and quality metrics.
Qualifications
- Active RN license in the practice state
- At least one year of clinical nursing experience
- Patient-focused, organized, and motivated to deliver high-quality care
- Reliable transportation, valid driver’s license, and CPR certification or ability to obtain
