Part Time Care Manager (PCA/CNA)
About the role
TerraBella Knightdale is a leading provider of healthcare, elder care. Our mission is to provide compassionate and comprehensive care to our clients, ensuring their well-being and quality of life. We are currently seeking a dedicated and passionate Care Manager to join our team during our 3rd shift and weekends.
Key Responsibilities
Client Care: Assess and identify clients’ needs through direct observation and interaction, providing personalized care and support.
Coordination: Collaborate with healthcare providers, family members, and interdisciplinary teams to create and implement individualized care plans.
Monitoring: Regularly monitor clients’ health status, documenting changes and communicating effectively with relevant parties.
Support: Offer emotional and psychological support to clients and their families, addressing any concerns or questions they may have.
Crisis Management: Respond to emergency situations with professionalism and care, employing conflict resolution and de-escalation techniques as necessary.
Documentation: Maintain accurate and up-to-date client records, ensuring compliance with regulatory standards and internal policies.
Education: Educate clients and families on care techniques, health management, and available resources to enhance their understanding and involvement in care.
Team Collaboration: Work closely with other staff members to create a positive and supportive environment that prioritizes client-centered care.
Qualifications
Education: Bachelor’s degree in Nursing, Social Work, Psychology, or a related field preferred. Relevant certifications and licenses (e.g., RN, LPN, Care Manager Certification) are a plus.
Experience: Previous experience in care management, nursing, or a related field, particularly in overnight or weekend settings is highly desirable.
Skills: Strong interpersonal and communication skills, with the ability to build rapport with clients and families.
Technology Proficiency: Experience working with electronic health records (EHR) and other healthcare technology.
Certifications: CPR and First Aid certified.
Availability: Ability to work overnight shifts and weekends consistently.
