This role supports Population Health initiatives through patient outreach and engagement, focusing on recruitment, retention, and strategies that improve health outcomes. The Outreach Coordinator connects patients with NEMS services, promotes preventive care, and increases access to culturally and linguistically appropriate healthcare resources. Strong communication and relationship-building skills are essential for working with patients, families, care teams, and community organizations to improve care coordination and address barriers.
Responsibilities
- Conduct outreach to newly assigned patients to establish primary care services and connect them with appropriate resources at NEMS sites.
- Perform targeted outreach to support population health initiatives, including annual wellness visits, preventive screenings, chronic disease management, and care gap closure.
- Engage diverse patient populations to promote NEMS services, programs, and population health initiatives while supporting marketing and community engagement strategies.
- Identify and understand the needs, interests, and barriers affecting current and prospective patients to increase awareness and utilization of NEMS services.
- Assist with data collection and tracking of outreach activities, reporting relevant statistical and operational information to management.
- Track outreach outcomes related to quality measures and population health initiatives to support reporting and performance improvement.
- Participate in Performance Improvement and Continuous Quality Improvement activities, including trainings and meetings.
- Ensure compliance with all applicable local, state, and federal regulations, as well as organizational policies and procedures.
- Perform other duties as assigned by manager or supervisor.
Requirements
- High school diploma with a minimum of 2 years of relevant work experience; Associate or Bachelor’s degree preferred.
- Experience with medical appointment scheduling required; Epic EHR scheduling strongly preferred.
- Prior experience in a managed care setting or IPA provider organization is a plus.
- Customer service experience with the ability to build rapport with patients via phone.
- Proficient with Microsoft Office and PC-based software or automated database management systems.
- Ability to follow through with delegated tasks and self-manage in a detail-oriented environment.
- Strong organizational and prioritization skills to manage multiple projects and timelines.
- Excellent interpersonal skills to collaborate with a broad range of internal and external stakeholders.
- Strong communication (spoken and written) and time management skills.
Language
- Must be fluent in English (spoken, read, and written).
- Fluent in Chinese (Cantonese and/or Mandarin) required.
- Fluency in other languages is an asset.

This is a full-time, non-exempt position.
