Provide primary care services to a diverse patient population, working collaboratively with other providers to manage patient health, perform assessments, and support community health initiatives.

Responsibilities

• Deliver comprehensive primary care to patients
• Adapt to diverse patient needs and cultural backgrounds
• Work effectively within a multidisciplinary team
• Perform health assessments and manage patient care plans
• Support and participate in community health initiatives

Requirements

• Minimum of 2+ years' experience in Primary Care, Urgent Care, or Community Health
• Licensed as an FNP or Physician Assistant in New Jersey
