Outpatient Behavioral Health Consult Liaison Care Navigator, Behavioral Health Services (Full Time)

Responsibilities

• Responsible for the review of daily consults.
• Reviewing and coordinating the most appropriate level of clinician to respond.
• Triage acuity of consults daily to help determine the order of response by C‑L clinicians.
• Act as the primary contact for the C‑L team to improve access to services.
• Assist with referrals to Inpatient Behavioral Health for IV ABX treatment.
• Assess patient needs and meet with patients to review unit and treatment expectations while admitted to IP Behavioral Health.
• Coordinate referrals to outside facilities and local community resources/organizations.
• Demonstrate the ability to work under pressure to meet deadlines, maintain accuracy, and handle constantly changing situations.
• Demonstrate the ability to interact with a variety of people, manage stressful situations, and handle conflict.
• Perform all job duties or tasks as assigned.
• Follow and adhere to all requirements, regulations, and procedures of any licensing board or agency.
• Comply with all Benefis Health System organization policies and procedures.

Requirements

• Current certification or licensure as a Certified Medical Assistant, Licensed Practical Nurse, Registered Nurse, or related Human Services field.
• Prior case management/navigator experience desired.

Pay

Non‑exempt position. Starting wage $24.12 per hour. Actual offered wage is based on applicable experience.
