Outcomes Manager, Inpatient Rehabilitation

About the role

Baptist Memorial Rehabilitation Hospital is a 53-bed inpatient acute rehabilitation hospital located in Germantown, Tennessee, part of Lifepoint Health, a diversified healthcare delivery network.

Responsibilities

• Coordinate PPD data collection with the interdisciplinary team.
• Ensure supportive documentation for QIs is complete in the medical record.
• Complete admission and discharge IRF-PAIs within CMS-designated timeframes.
• Abstract required information from medical records and summarize on the IRF-PAI.
• Maintain and update the IRF-PAI log; verify accuracy against the patient location roster.
• Perform other duties as assigned.

Requirements

• Current clinical degree in OT, PT, RN, SLP, or MSW.
• QI competency.
• Familiarity with state and federal guidelines.
• Familiarity with healthcare workflows and documentation.

• Strong clinical assessment/analysis skills.
• Effective verbal and written communication.
• Excellent interpersonal skills.
• Ability to collaborate across disciplines and meet strict deadlines with high accuracy.

The pay for this position is commensurate with experience.
