Orthopedic At-Home Physical Therapist - Per Diem

About the role

We are seeking a compassionate Orthopedic At-Home Physical Therapist to evaluate and treat patients recovering from surgeries and orthopedic conditions in their homes.

Responsibilities

• Conduct initial assessments of patients in their homes.
• Develop and implement individualized treatment plans.
• Educate patients on safe mobility and transfers.
• Monitor patient progress to facilitate safe discharge and community reintegration.
• Document care using EMR software.
• Collaborate with interdisciplinary teams.
• Ensure safe and effective interventions aligned with best practices.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy (Doctorate preferred).
• Current state licensure.
• Completed continuing education unit (CEU) requirements.
• CPR certification.
• Strong communication and decision-making skills.

Key duties

• Patient education on mobility and recovery.
• Functional training and exercise prescription.
• Maintaining accurate records for billing and regulatory compliance.
