Per Diem Orthopedic Home Physical Therapist

Responsibilities

• Evaluate and treat patients recovering from orthopedic surgeries, such as total hip and knee replacements, in their homes.
• Educate patients on safe mobility, transfers, and ambulation.
• Develop and implement individualized treatment plans.
• Perform functional training.
• Document care, progress, and discharge plans.
• Collaborate with interdisciplinary teams.
• Maintain accurate records and ensure safe practices.
• Provide patient education to promote recovery and community reintegration.

Qualifications & Skills

• Bachelor’s or Master’s degree (Doctorate preferred).
• Valid state licensure.
• CPR certification.
• Experience with EMR software preferred.
• Strong communication, clinical decision-making, work ethic, and teamwork skills.
• Basic computer proficiency required.
