Orthopedic Physical Therapist - Per Diem

About the role

We are seeking a Per Diem Orthopedic At-Home Physical Therapist to evaluate and treat patients recovering from orthopedic surgeries and conditions in their homes.

Responsibilities

• Educate patients on safe mobility, transfers, and ambulation
• Conduct evaluations and develop individualized treatment plans
• Provide functional training for activities of daily living and community reintegration
• Document care using EMR software
• Communicate with the care team
• Ensure patients are ready for safe discharge

Requirements

• Bachelor’s or Master’s Degree in Physical Therapy (Doctorate preferred)
• Valid state licensure as a Physical Therapist
• CPR certification
• Familiarity with EMR systems
