Physical Therapist - Per Diem

About the role

Assess patient needs, develop individualized treatment plans, and perform interventions to improve functional status, mobility, and pain management in a home-health setting.

Responsibilities

• Evaluate patients' functional status and assist in creating therapy plans in collaboration with physicians.
• Provide therapy services within legal and professional scope, coordinating with the healthcare team.
• Monitor and report patient responses and condition changes.
• Treat patients to relieve pain, restore function, and maintain maximum functioning.
• Educate patients, families, and healthcare team members regarding care plans and progress.
• Complete patient evaluations, progress notes, and develop plans according to state guidelines.
• Report outcomes, goals, and projected care frequency.
• Adapt to diverse home environments, ensuring equitable care regardless of background.

Requirements

• Current Physical Therapy license in the practice state.
• Valid driver’s license, vehicle insurance, or reliable transportation.

Qualifications

• Preferred CPR certification or ability to obtain within 90 days.
