Responsibilities
- Provide clinical assessment, intervention, and case management services to older adults experiencing physical, cognitive, emotional, or social challenges.
- Conduct mental health and wellness assessments and develop individualized service plans.
- Deliver crisis intervention, short-term supportive counseling, and ongoing clinical monitoring as appropriate.
- Provide advocacy and care coordination, including referrals to medical, behavioral health, financial, housing, and community-based resources.
- Maintain timely, accurate, and ethical clinical documentation in alignment with professional standards.
- Provide clinical leadership and oversight of the Friendly Visiting Program, ensuring appropriate matching, boundaries and risk management practices.
- Plan, coordinate, and evaluate a diverse year-round calendar of social, educational, wellness, recreational, spiritual, and intergenerational programs that promote healthy aging, lifelong learning, and meaningful community engagement.
- Manage program operations by coordinating schedules, transportation, volunteers, facilities, budgets, supplies, and resources to ensure high-quality, mission-aligned programming.
- Promote and enhance participant engagement through community outreach, partnerships, and ongoing evaluation of program outcomes and participant feedback to continuously improve services.
- Collaborate with Volunteer Services to screen and assess volunteers, assess potential risk factors, and ensure proper training in trauma-informed and elder-sensitive care.
- Supervise and mentor interns (BSW/MSW level), providing clinical guidance, case consultation, and performance evaluations.
- Partner with community agencies, healthcare providers, and housing managers to strengthen referral pathways and support continuity of care.
- Contribute to program evaluation, outcome tracking, and strategic development of a comprehensive senior services model in collaboration with executive leadership.
Requirements
- Master’s degree and clinical license in social work or related field (LCSW or LCPC).
- Previous experience working with older adults- a sensitivity to the needs of older adults and ability to navigate the various systems surrounding work with older adults.
- Sensitivity to the needs of a low-income, culturally diverse population.
- Previous experience in the nursing or medical field is a plus.
- Valid Illinois driver’s license, reliable transportation, and safe driving record.
- Able to lift, move and carry event supplies (up to 25lbs).
