About the role

Provide personalized, one-on-one occupational therapy to older adults in private homes and senior living communities, supporting mobility, independence, and quality of life through evidence-based treatments.

Responsibilities

• Perform evaluations and develop care plans
• Educate patients, caregivers, and staff to promote safety and aging in place
• Document care accurately within electronic medical records
• Collaborate with an interdisciplinary team of health professionals
• Incorporate telehealth and virtual care to enhance flexibility and patient access

Qualifications

• Valid license to practice occupational therapy
• Degree from an accredited occupational therapy program
• Proficiency in EMR and basic computer skills
• CPR certification
• Physically capable of performing tasks such as lifting, repositioning patients, and driving
