About the role

Provide personalized, one-on-one occupational therapy to older adults in private homes and senior living communities, enhancing mobility, independence, and quality of life.

Responsibilities

• Conduct evaluations and develop individualized care plans
• Deliver evidence-based treatments
• Document care in electronic medical records (EMR)
• Collaborate with interdisciplinary teams
• Educate patients, caregivers, and staff to promote safety and aging in place
• Incorporate telehealth and virtual care to increase flexibility and patient access

Qualifications

• Valid Occupational Therapy license
• Degree from an accredited Occupational Therapy program
• Basic computer skills
• Current CPR certification

Physical demands

• Lifting and repositioning patients
• Assisting with transfers
• Driving to patient locations
