Role Overview

Seeking a licensed Occupational Therapist to provide personalized, one-on-one care to older adults in private homes and senior living communities. The role involves conducting evaluations, developing individualized treatment plans, and delivering evidence-based therapy to enhance mobility, independence, and overall quality of life. Therapists may incorporate telehealth and virtual care to increase flexibility and patient access.

Key Responsibilities

• Perform assessments and create tailored care plans
• Administer therapy sessions supporting wellness and functional independence
• Document care within EMR systems, ensuring regulatory compliance
• Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Utilize telehealth when appropriate to extend care access

Qualifications & Skills

• Valid OT license or eligibility in practice state
• Degree from an accredited OT program
• Proficiency with computers and EMR systems
• Current CPR certification
• Ability to meet physical demands such as lifting, repositioning, and extended walking
