Role Overview
An Occupational Therapist provides personalized, one-on-one care to older adults in their homes or senior living communities, supporting mobility, independence, and overall well-being. They develop individualized treatment plans, conduct evaluations, and deliver evidence-based therapies to enhance patient function and safety.
Key Responsibilities
- Deliver in-home and community-based occupational therapy sessions, including virtual care when appropriate
- Perform assessments, create care plans, and implement therapeutic interventions
- Document patient progress accurately within electronic medical records
- Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff on safety, aging in place, and functional independence
Qualifications & Skills
- Valid occupational therapy license or eligibility in practice state
- Degree from an accredited OT program
- Proficiency in basic computer use and EMR systems
- Current CPR certification
Physical & Other Requirements
Ability to perform moderate physical activities, assist with patient transfers, and drive to various locations. Must be able to wear PPE when needed.
