About the Role

The Occupational Therapist provides personalized in-home and senior living care to older adults, focusing on improving mobility, independence, and quality of life through evidence-based treatment and assessments.

Responsibilities

• Deliver one-on-one therapy sessions in various settings, including private residences and senior communities
• Perform evaluations, develop individualized care plans, and implement therapeutic interventions
• Document patient progress accurately within electronic medical records
• Collaborate with interdisciplinary teams to promote patient wellness and safety
• Educate patients, caregivers, and staff on safety and aging-in-place strategies
• Utilize telehealth and virtual care to increase access and flexibility

Requirements

• Valid occupational therapy license or eligibility in practice states
• Degree from an accredited OT program
• Proficiency in computer use and EMR systems
• Current CPR certification
• Ability to perform physical tasks such as lifting, repositioning patients, and driving
• Ability to wear PPE when necessary
