About the role
The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to improve or adjust their daily activities.
Responsibilities
- Collaborate with healthcare teams, document care details, and update goals based on patient progress
- Educate patients and families, assess home safety, recommend equipment, and manage supplies
- Supervise and instruct Occupational Therapy Assistants and Home Health Aides, ensuring compliance with regulations and proper documentation
- Conduct comprehensive assessments, communicate with physicians, and plan discharge procedures
Qualifications
- Bachelor's Degree in Occupational Therapy
- Licensure as an Occupational Therapist
- BLS certification
- Valid driver's license with auto insurance
