Occupational Therapist (OT) - Home Health
About the Role
SummitWest Care is a local, nonprofit home health agency seeking a compassionate, skilled Occupational Therapist (OT) to provide one-on-one care to patients throughout Mesa County. This position offers the opportunity to build meaningful relationships while helping individuals safely navigate daily life in the comfort of their own homes. You'll work independently in the field with the full support of a collaborative, interdisciplinary team committed to delivering high-quality patient-centered care. As a home health OT, you'll have the autonomy to focus on clinical outcomes, patient education, and individualized treatment plans without the pressure of unrealistic productivity expectations.
Responsibilities
- Perform occupational therapy evaluations, reassessments, recertifications, and discharges
- Develop and implement individualized treatment plans focused on safety, function, and independence
- Assess patients' ability to perform ADLs
- Conduct home safety evaluations and recommend adaptive equipment or environmental modifications
- Educate patients and caregivers on home programs, mobility strategies, and safety techniques
- Monitor patient progress and adjust treatment plans as appropriate
- Collaborate with nurses, therapists, caregivers, physicians, and family
- Supervise and support COTAs when applicable
- Complete accurate and timely clinical documentation
Requirements
- Current Colorado Occupational Therapist (OT) license in good standing
- Graduate of an accredited Occupational Therapy program
- Strong clinical assessment, communication, and documentation skills
- Ability to work independently in a home health environment
- Excellent time-management and organizational skills
- Valid driver's license, reliable transportation, and auto insurance
Home health experience is preferred but not required. We are willing to train the right candidate!
