Role Overview

The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages to improve or adjust daily activities. They collaborate with the healthcare team, educate patients and families, and document care comprehensively to ensure continuity.

Responsibilities

• Reviewing care plans
• Providing patient education
• Ordering supplies
• Assessing home safety
• Supervising therapy assistants
• Conducting comprehensive evaluations
• Managing schedules
• Communicating with physicians and caregivers to support discharge planning and ongoing care
• Ensuring compliance with regulations and standards
• Completing audits
• Maintaining accurate documentation

Qualifications & Skills

• Licensed Occupational Therapist (OT) with valid state license
• CPR/BLS certification within 30 days
• Valid driver's license and proof of auto insurance
• Bachelor's Degree in Occupational Therapy
• Strong assessment, critical thinking, and communication skills
• Knowledge of home care regulations and documentation standards
