Occupational Therapist (OT) - Home Health

About the role

We are seeking a Home Care Occupational Therapist to evaluate and develop treatment plans for patients of all ages, aiming to improve or adjust daily living activities.

Responsibilities

• Reviewing and coordinating care plans with physicians and documenting thoroughly.
• Educating patients and families, ensuring understanding and adherence to therapy goals.
• Assessing patient function, home safety, and recommending necessary equipment.
• Supervising and instructing therapy assistants and home health aides.
• Completing comprehensive evaluations, documentation, and regulatory assessments such as OASIS.
• Managing supplies, discharge planning, and communication with healthcare team members.

Qualifications

• Licensed Occupational Therapist with valid state licensure.
• Current BLS certification within 30 days.
• Valid driver’s license and auto insurance.
• Bachelor’s degree in Occupational Therapy.
• Strong documentation, communication, and patient education skills.
