Role Overview

The Home Care Occupational Therapist evaluates and develops treatment plans for patients of all ages, focusing on resuming or adjusting daily activities. They collaborate with the healthcare team, modify care plans as needed, and serve as a clinical resource and role model for staff and students. The therapist conducts comprehensive home assessments, educates patients and families, and ensures proper documentation of care, goals, and discharge planning. They evaluate patient function, recommend home safety modifications, and guide other health team members, including Occupational Therapy Assistants and caregivers. The role requires supervising OTA visits, completing assessments like OASIS, and maintaining compliance with home care regulations.

Qualifications & Skills

  • Bachelor's Degree in Occupational Therapy
  • Licensed Occupational Therapist (IDFPR)
  • Basic Life Support (BLS) certification
  • Valid Driver's License and auto insurance
  • Ability to work collaboratively in an interdisciplinary team
  • Strong assessment, documentation, and patient education skills
  • Knowledge of home care rules, regulations, and safety protocols

Key Responsibilities

  • Review and communicate care plans with physicians
  • Educate patients and families, addressing barriers and needs
  • Document visits, assessments, and discharge plans accurately and timely
  • Order supplies, manage caseload, and coordinate scheduling
  • Assess home safety and recommend modifications
  • Supervise OTAs and home health aides, ensuring quality care
  • Complete comprehensive evaluations and assessments, including OASIS
  • Maintain compliance with regulatory standards and documentation requirements
