Occupational Therapist (OT) - Home Health

The role involves evaluating and developing treatment plans for patients of all ages to improve or adjust daily activities, collaborating with healthcare teams, and serving as a clinical resource and role model. Responsibilities Include Reviewing care plans and orders with physicians, documenting assessments, and updating goals based on patient progress.Educating patients and families, ensuring clear communication and overcoming barriers.Documenting care thoroughly to guide future treatments, including discharge planning and resource coordination.Evaluating patient function, guiding therapeutic activities, and monitoring treatment reactions.Instructing and supervising occupational therapy assistants and home health aides, ensuring compliance with home care regulations.Performing comprehensive assessments, including safety evaluations and home modifications recommendations. Qualifications And Skills Licensed Occupational Therapist with applicable state licensure and BLS certification.Bachelor’s degree in Occupational Therapy.Experience in home care is preferred; sign-on bonuses available based on experience.Strong communication, documentation, and critical thinking skills, with knowledge of home care rules and regulations.
