Occupational Therapist (Home Health)

About the Role

Provides occupational therapy services aimed at restoring and enhancing patients' physical, social, and mental functions within a collaborative, interdisciplinary team.

Responsibilities

• Conducts evaluations to develop personalized treatment plans
• Implements therapy interventions using established protocols
• Communicates discharge and care plans effectively
• Completes accurate clinical documentation within deadlines
• Supervises assigned therapy assistants
• Performs additional related duties as needed

Requirements

• Master's degree or Doctor of Occupational Therapy (OTD)
• Licensure in applicable states
• Valid occupational therapy license
• CPR/BLS certification
• Valid driver’s license, with a minimum age of 21
• Must adhere to organizational safety policies, including vaccination requirements
