Occupational Therapist (Home Health)

Responsibilities

• Assess and evaluate patients' functional status, activities of daily living, and occupational therapy needs in accordance with state practice laws and care plans
• Conduct home assessments and recommend adaptive equipment as needed
• Collaborate with physicians and healthcare team members to develop and modify therapy plans
• Monitor and document patient responses to treatment and any condition changes
• Apply evidence-based techniques within the scope of practice to improve patient outcomes
• Provide patient-centered care, evaluate functional abilities, document progress, and ensure compliance with healthcare standards

Requirements

• Current Occupational Therapy licensure in the state
• CPR certification
• Valid driver’s license, reliable transportation, or access to public transit
• Ability to work effectively in diverse home environments
