About the role
Provide therapeutic services in home health settings to promote independence and improve quality of life for patients.
Responsibilities
- Assess patient needs and develop individualized treatment plans
- Implement evidence-based interventions to address physical, cognitive, and psychosocial challenges
- Collaborate with interdisciplinary teams to coordinate care
- Evaluate progress and adjust treatment plans as needed
Requirements
- Bachelor’s degree in Occupational Therapy from an accredited program
- Current state license as an Occupational Therapist
- At least 1 year of relevant work experience
Qualifications
- Strong clinical skills and knowledge of occupational therapy principles
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Skills
- Proficient in using medical software and documentation systems
- Knowledge of community resources and support services
