About the role

As a Home Health OT, you’ll help patients regain independence and learn to perform everyday activities. You’ll work with a home health team—including RNs, LPNs, CNAs, HHAs, PTs, and SLPs—to focus on each patient’s plan of care and personal goals. Your role will involve assessing patients, observing deficits, establishing therapy goals, and documenting progress.

Responsibilities

  • Provide occupational therapy to patients unable to perform daily tasks due to illness or injury.
  • Assess patients, observe deficits, establish therapy goals, and document progress.
  • Assist patients with exercises to improve fine motor skills and coordination.
  • Suggest adaptive equipment such as grab bars and shower chairs for added support.
  • Assess fall risks and introduce strategies to improve home safety.
  • Educate patients and families on the plan of care, exercises, goals, and self-care.
  • Maintain accurate and up-to-date documentation in accordance with agency policies and regulations.

Requirements

  • Graduate of an accredited Occupational Therapy Program with an active OT license in South Carolina.
  • Minimum of 1 year of occupational therapy experience, ideally in home healthcare.
  • CPR certification.
  • Valid car insurance.
  • Knowledge of state and federal home health regulations.
  • Good clinical judgment, strong interpersonal skills, resourcefulness, and compassion.
