About the role

Continuum Therapy Partners is seeking a compassionate and dedicated Occupational Therapist (OT) to join our rehabilitation team at Sauk County Health Care Center in Reedsburg, Wisconsin. This opportunity offers Full-Time, Part-Time, and PRN scheduling options for a licensed Occupational Therapist who enjoys providing meaningful, patient-centered care in a supportive skilled nursing environment.

Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services in a skilled nursing facility setting.
• Address activities of daily living (ADLs), functional mobility, coordination, strength, cognition, and safety awareness.
• Help patients achieve greater independence and participate in meaningful daily activities.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety strategies.
• Deliver compassionate, evidence-based occupational therapy services.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Wisconsin Occupational Therapist (OT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
